I hereby affirm that I give free consent for my son/daughter to be minded by St Cecilia’s Childcare & Kindergarten staff at the Tarxien Childcare Centre and Kindergarten School (hereinafter referred to as the “Centre”), during the hours specified, subject to the booking being confirmed by the administration.

I appreciate that there are inherent risks. I understand and agree that neither the Childcare Educators, other members of staff, nor St Cecilia Tarxien Ltd (hereinafter referred to as the “Released Parties”), may be held liable or responsible in any way for any injury, death or other damages to me, my family, estate, heirs or assigns, that may occur as a result of the children partaking in activities at the Centre, or as a result of the negligence of any party, including the Released Parties, whether passive or active. I hereby personally assume responsibility for all risks whether foreseen or unforeseen, that may befall my son/daughter while they are being minded at the Centre. I further release, exempt and hold harmless said activities and Released Parties from any claim or lawsuit by me, my family, estate, heirs or assigns, arising out of my son/daughter’s enrolment in the Child-minding service.

I further state that I am legally competent to sign this liability release on behalf of my son/daughter and any accompanying children mentioned in this booking request. I also confirm that I am authorised to leave said children in the care of St Cecilia’s Childcare and Kindergarten staff, having obtained suitable permission from parents/guardians where the children are not my own. 

In addition, I give St Cecilia’s Childcare and Kindergarten Staff permission to act in loco parentis (instead of the parents) should the need arise, while the children are in their care. In case of emergency, I give said staff permission to administer First Aid, and to take the necessary steps which may include, but are not limited to; taking the children to the nearest doctor, clinic or hospital, or calling for help in order that the best interests of the children are sought.

I understand that the terms herein are contractual and not a mere recital and that I agree to these terms of my own free act and with the knowledge that I hereby agree to waive my legal rights. I further agree that if any provision of this agreement is found to be unenforceable or invalid, that provision shall be severed from this Agreement. The remainder of this Agreement will then be construed as though the unenforceable provision had never been contained herein.
